Short Story
Hello, my name is Henry Lopez and I have been recently unemployed for about a month now. The reason being is I missed a lot of work due to doctor visits and some even being from 911 calls; because my wife came home and found me unresponsive. Another time I had a seizure and my sugar went as low as 20. I am diabetic with high blood pressure and on medications. The reason I’m barely asking for help is because I thought I would be able to manage the bills because my wife got a part time job but we have been so broke that we had to borrow money from her family and my prior job didn’t honor my doctors notes because they said they don’t have a position for “light duty” work. I understood because the work I did was fast pace and vigorous heavy lifting. We are in debt over $700.00, not including all the people we owe from borrowing. Our family knows that we are doing this and even told us that we can take as long as it takes to pay them back. However, after Thanksgiving we will be short on the rent and food. We applied for assistance from the county and that should help out soon. As of now I hope someone can come to our rescue. I thought I be accountable and attach my prior visit from the hospital.
HISTORY OF PRESENT ILLNESS (HPI)
Henry T Lopez is a 45 year old male with past medical history DM II and hypertension presents with hypoglycemia and seizure.
Was admitted for metabolic encephalopathy
Current Outpatient Medications:
*metFORMIN 500* Insulin NPH & Regular Human (HUMULIN 70/30 U-100 KWIKPEN) 100 unit/mL SubQ Insulin Pen.
* Empagliflozin (JARDIANCE) 25 mg Oral Tab Take onehalf tablet by mouth daily to lower sugar and/ protect heart and kidneys.
*Rosuvastatin (CRESTOR) 10 mg Oral Tab, Take 1 tablet by mouth daily to reduce cholesterol risk of heart attacks and strokes.
